New Student Enrollment
In order to enroll your student at Evergreen Middle School, you will need to fill out a registration packet (available in the office) and provide:
- Birth Certificate
- Proof of Immunizations
- 2 Recent Proofs of Current Residency
When you come in to pick up a registration packet, please bring all of the above with you. Registration will not be complete until all items have been provided to the office.
